Huon Regional Care (HRC) supports older people to live well at home across Southern Tasmania. We’re a values-led organisation and we’re looking for an experienced Service Manager – Support at Home to lead day-to-day service delivery and team performance.
The role
Reporting to the Executive Manager Support at Home, you’ll oversee intake, care management, scheduling efficiency, quality/compliance, and continuous improvement across our Support at Home and CHSP services. You’ll lead Care Partners and Service Coordinators, and work closely with clinical and allied health professionals and associate providers.
What you’ll be responsible for
• Leading service planning, intake, care management and review
• Improving scheduling efficiency, coverage and travel optimisation
• People leadership: performance, coaching, workforce planning and recruitment support
• Quality, risk and compliance: audits, incidents, complaints, feedback and improvement actions
• Budget oversight: client budgets/pooled care management minutes and operational performance
About you
• Proven leadership in community/aged care service delivery (3–5+ years)
• Strong knowledge of aged care programs, funding and compliance
• Confident using data to improve outcomes and efficiency
• Great communicator with a practical, solutions-focused approach
Requirements
• Qualification in Aged Care/Nursing/Health Services/Social Work/Business
• Current National Police Check
• Registration to Work with Vulnerable People (incl. NDIS endorsement)
• Current driver’s licence
• Desirable: RN/EN qualification
