Forming Connections is a neuro-affirming, trauma-informed NDIS provider committed to building meaningful relationships, strengthening systems around people, and supporting practitioners to do their best work—without burning out. This role is ideal for practitioners who value flexibility, ethical practice, and strong supervision.
We are seeking a person-centred and clinically minded Behaviour Support Practitioner to join our Southeast Queensland team. We already have an Advanced Behaviour Support Practitioner based in North Brisbane who services the surrounding regions, and we are now looking to expand to take on new referrals.
The successful candidate will be responsible for providing support to NDIS participants, their families, and carers with behaviours of concern, helping them to develop coping strategies and improve their quality of life. The role requires a compassionate, patient individual who can work collaboratively with clients, families, schools, Support Coordinators, NDIS providers, and other healthcare professionals.
About the role:
- Conduct assessments to identify the underlying functions of behaviours and unmet needs
- Develop and implement evidence-based, rights-focused behaviour support plans tailored to individual, family, and system needs
- Provide training and support to clients, families, and caregivers on behaviour management strategies
- Use a capacity-building, skills-focused approach that prioritises quality of life and reduction of restrictive practices
- Monitor progress and adjust support plans as necessary
- Collaborate with other healthcare professionals to ensure a holistic approach to care
- Maintain accurate and up-to-date records of progress and interventions
- Attend team meetings and participate in case conferences as required
- Work autonomously from home and in the community, while remaining closely connected to a supportive clinical team
Who we’re looking for:
- A suitable Behaviour Support Practitioner (Core, Proficient, or Advanced) (Highly desirable)
- Hold relevant qualifications (Psychology, Social Work, OT, SLP, Education, Counselling, or similar)
- Have experience supporting people with intellectual disability, autism, psychosocial disability, or complex support needs
- Write clear, respectful, high-quality reports aligned with NDIS requirements
- Practise in a trauma-informed, neuro-affirming, and person-centred way
- Value reflection, supervision, and continuous learning
- Compassionate and patient approach to working with others
- Are organised, reliable, and comfortable working remotely
