About the Role:
As a Care Coordinator, you’ll play a pivotal role in supporting our participants to live safely and happily at home. You’ll be responsible for but not limited to:
- Creating and updating individualised care plans , conducting annual reassessments and managing participants’ budgets.
- Supporting participants with a model of care that focuses on their goals, independence, and wellbeing and reablement.
- Ensuring care aligns with Aged Care Quality Standards and the Statement of Rights
- Monitoring service delivery and making sure participants have the right level of funding and support
- Management of Support staff

FOCUS Connect seeks to ensure that our organization and our services are relevant and accessible. Based on an integrated service delivery system FOCUS Connect’s programs are specifically tailored through 4 broad service areas: children, youth, family and aged.
What you will bring to the role:
- A strong working knowledge of the Aged Care Quality Standards (including the Aged Care Reforms – July 2025)
- Experience managing a caseload in a participant-focused environment
- Excellent communication skills and a genuine ability to connect with people
- Well-developed organisational and time management abilities
- Confidence in conducting assessments and developing personalised care plans
- A sound understanding of the Support at Home program
What You’ll Need:
- Bachelor’s degree in Nursing, Social Work, or related field (preferred) A teritary qualification is efficient for the role.
- Minimum of 2 years’ experience in Aged Care coordination or related role
- Current driver’s license and reliable vehicle
- Current and cleared; National Criminal History Check and WWCC (or willing to obtain)
- Current and valid First Aid and CPR certificate
