Description

Focus Care is seeking an experienced and passionate Support Coordinator to join our team. In the role as a Support Coordinator, you will be supporting NDIS participants across NSW, Victoria and Queensland.

The Support Coordinator is responsible for supporting NDIS participants by providing effective case management through efficient use of their NDIS plans and funding.

About the Role

  • Develop and maintain trust and rapport with participants and their support networks
  • Supporting participants to manage their NDIS funding to ensure it adequately meets their support needs
  • Building the participants, and/or informal supports’, capacity to advocate for themselves and navigate the NDIS more confidently
  • Critically analysing participants’ support needs to identify gaps in their service provision, highlighting relevant referrals to other allied health professionals
  • Completing implementation and review reports to support participants maintain/increase their funded supports and comply with NDIS Standards.

About you

  • Qualification in Allied Health or related discipline.
  • Sound understanding and knowledge of NDIS frameworks and funding.
  • Minimum 3 years’ experience in a similar role.
  • High-level of organisational, time management and planning skills
  • Strong problem-solving skills
  • Excellent communication (verbal and written)
  • Valid full driver’s license (own vehicle is not required)
  • NDIS Worker Check (or willingness to obtain before commencement)
