Description

We are looking for a passionate and collaborative (full-time or part-time) Enrolled Nurse to join our small, dynamic Clinical Team.

Reporting to the Quality and Clinical Manager, this role will provide care to our clients in their homes, enabling them to live their best lives.

Whilst this is an office-based role, you will regularly meet with clients in their own home and under supervision from clinicians, assess their needs and implement a preventative based approach to health care through clinical skill and knowledge.

Key responsibilities of the role include:

  • Under supervision, conducting clinical assessments of clients in their homes
  • Identifying clinical care needs and taking appropriate action
  • Liaising with client’s care team (family; GP; allied health service providers) to ensure a coordinated approach to client care.
  • Initiating appropriate coaching with clients for their self-care needs
  • Conducting training for Support Workers for their clinical skill development
  • Performing nursing care with clients with complex needs
  • Review of clinical incident management and report outcomes and recommendations to management.

About You

  • Enrolled nurse or working towards tertiary qualification in Nursing and current registration with AHPRA – Australian Health Practitioner Regulation Agency.
  • Experience in working within the health, community, or aged-care and disability sector
  • Knowledge of care plan implementation, wound care and management, medication support, continence assessments and management, clinical monitoring, nursing advice and recommendations and general nursing duties in conjunction with personal care plan.
  • Demonstrated ability to work efficiently in an autonomous capacity.
  • Superior communication skills to promote collaboration within the multi-disciplinary team, Focus Care and other organisations as necessary for provision of optimum client care.
  • Excellent time management and organisation skills.
  • High level of computer literacy and effective and efficient data management and data entry.
  • Valid and current Driver’s License
  • Current First Aid and CPR Certificate and National Police Check
  • A readiness to travel between the office and clients in the performance of your role (company car will be provided for all travel to and from clients)
