About the Role

We’re looking for a Care Coordinator to join our team in Brisbane. You’ll be the key contact for Members, coordinating the assessment, planning, and delivery of care and services—both through our Helpers (support workers) and trusted third-party partners.

This is a role where empathy meets expertise. You’ll work closely with Care Specialists, Community Nurses, and Workforce Coordinators to create tailored solutions that help Members live well at home. From updating Help Plans to arranging allied health referrals, every day brings new opportunities to make a meaningful impact.


What You’ll Be Doing

  • Coordinating care plans and services for Members, ensuring quality and continuity.
  • Engaging with Members and families to understand needs and goals.
  • Updating Help Plans and advocating for additional funding when required.
  • Referring to internal and external providers for clinical and allied health support.
  • Using our software to manage workflows and help Members navigate their care.
  • Collaborating with the Care Team to deliver holistic, person-centred solutions.
  • Responding with empathy and creativity—whether it’s arranging extra support, sending flowers, or connecting carers to resources.


About You

  • Minimum 2 years’ experience as a Care Coordinator in Support At Home packages or similar.
  • Skilled at maximising package budgets and leveraging community supports.
  • Strong communicator—both written and verbal—with a down-to-earth approach.
  • Patient, empathetic, and able to build trust with Members and families.
  • Highly organised with an eye for detail and deadlines.
  • Comfortable using technology and advocating for its benefits.
  • Collaborative team player who values feedback and continuous improvement.
