About the role

As an Enrolled Nurse or Registered Nurse at Hope and Change, you will provide clinical and care support to people with disability living in their homes, including those with complex health, personal care and behavioural support needs. Depending on the role and your scope of practice, you may be involved in medication administration, clinical oversight, health monitoring, wound care, manual handling support, documentation, care plan implementation, staff guidance and liaison with families, allied health professionals and other stakeholders.

This role requires nurses who can work with professionalism, sound judgement and consistency, while contributing to safe, respectful and person-centred support in a home-based environment. You will be part of a team that values quality, accountability and practical clinical leadership.

Your availability across all shift types, including active overnight shifts and participation in an on-call roster, is essential.

Key responsibilities

  • Deliver safe, person-centred nursing and support services in clients’ homes and community settings
  • Provide nursing care and support with activities of daily living
  • Administer medication and monitor clinical responses
  • Support implementation of care plans and clinical instructions
  • Undertake manual handling and transfers safely
  • Complete accurate documentation, progress notes and incident reporting
  • Communicate effectively with families, clinicians and the broader support team
  • Participate in active overnight shifts and on-call arrangements as required

What you will bring

You will be an experienced Enrolled Nurse or Registered Nurse with strong clinical judgement, a professional approach and a genuine commitment to high-quality care. You will be confident working in complex and changing environments, understand the importance of safe escalation and accountability, and work well with clients, families and multidisciplinary teams.

Enrolled Nurses will bring solid practical experience, the ability to work confidently within scope, sound judgement in changing care environments, and the ability to recognise when escalation to a Registered Nurse is required.

Registered Nurses will bring the ability to provide comprehensive nursing care, coordinate services around client needs, support less experienced staff where required, and take accountability for clinical decision-making, service delivery and care outcomes.

Skills, competencies and mandatory training

To be successful in this role, you will have:

  • current AHPRA registration as an Enrolled Nurse or Registered Nurse
  • relevant experience in acute care, community nursing, complex care, case management and/or palliative care
  • strong communication skills and sound clinical judgement
  • the ability to practise in accordance with the standards applicable to your registration
  • experience in, or willingness to complete competency-based training and assessment in participant-specific supports

Relevant clinical competencies may include:

  • manual handling
  • medication administration and clinical monitoring
  • PEG / enteral feeding
  • tracheostomy care
  • ventilator support
  • urinary catheter and SPC management
  • continence and complex bowel care
  • colostomy / ileostomy and stoma care
  • seizure management
  • oxygen and nebuliser administration
  • suctioning
  • wound care and pressure injury management

Applicants must also hold, or be willing to obtain and maintain:

• current Australian driver licence

• current Blue Card

• current Yellow Card or NDIS Worker Screening Clearance, as applicable

• completion of the NDIS Worker Orientation Module

• completion of the NDIS Supporting Effective Communication module

• medication administration competency, including HLTHPS006 or equivalent

Where required, you must be willing to complete participant-specific training and competency assessment before working independently with particular clients.
