Summary:

We are seeking a highly skilled and compassionate Community Registered Nurse to join our team. The successful candidate will be responsible for providing high level of nursing care to our disability participants in their homes. The position of Community Registered Nurse focuses on a direct disability-related health service provision with NDIS participants, and quality and service improvement activities across the organisation focused on customers’ health and wellbeing.

Responsibilities:

  • Conducting comprehensive health assessments to understand the individual’s needs and developing individualized care plans.
  • Administering/supervising administration of prescribed medications and monitoring for any adverse effects or responses.
  • Providing a range of nursing interventions, treatments, and therapies as needed, such as wound care, catheter care, and continence management.
  • Regularly monitoring the individual’s health status, responses to treatment, and adjusting care plans as necessary.
  • Educating individuals and their families on managing their conditions, promoting healthy lifestyles, and preventing illness.
  • Working with other healthcare professionals, such as doctors, therapists, and support workers, to ensure coordinated and holistic care.
  • Maintaining accurate and up-to-date records of assessments, interventions, and outcomes, using electronic health record systems.
  • Working within the National Disability Insurance Scheme (NDIS) framework and home care settings.
  • Supporting individuals to maintain their independence and advocating for their needs.
  • Supervising and mentoring other healthcare professionals, such as enrolled nurses and support workers.
  • Participating in quality assurance activities and contributing to the development of policies and procedures.
  • Engaging in ongoing professional development to maintain and enhance their skills and knowledge.

Qualifications:

  • Bachelor’s degree in nursing or related field
  • Active Registered Nurse license
  • Minimum of 2 years of experience in a clinical setting
  • Strong communication and interpersonal skills
  • Ability to work in a fast-paced environment
  • Knowledge of NDIS regulations and standards
  • NDIS Worker Screening Check*
  • National Police Clearance
  • NDIS Orientation
  • Current CPR
  • Manual Handling
  • Current Drivers Licence
  • Working With Children’s Card
