psychosocial disabilities in supported living accommodation. We provide NDIS
participants with individual support needs with a range of supported

About the Role
We are looking for experienced Male Support Workers to join our team in Perth [North-east suburb] to support our participants in their community access and helping them achieve their goals. The aim of the NDIS is to give participants choice and control over what supports and services they need, how and by whom they are going to be provided.
Casual roles are available for this position.
The Duties of this role are:
- Positive Behavior Support strategies implementation.
- Able and willing to work short community shifts including weekend shifts
- Form and maintain good rapport and working relationships with participants to
help them achieve their goals
- Build participants’ capacity to live as independently as possible. This includes
supporting participants with their daily living needs, physical health and
well-being, community participation where participants are given
opportunities to make meaningful social connections, to explore work
and/or educational opportunities
- Engage and support family members and/or carers in participants’ recovery process
- Provide information and refer participants to service/s where appropriate
- Establish and maintain close working links with relevant mental health and NDIS
service providers to optimise participant referral pathways, ensure inefficient and quality supports for participants
Selection Criteria
We are looking for someone with:
- 2-3 years experience in mental health and disability support
- Significant experience with mental health and/or complex disabilities.
- Understanding of/experience with the NDIS practice guidelines and standards
- Medication competency
- A professional commitment to making a difference in the lives of participants, their family members and/or carers
- A proven capacity to build rapport with participants from a diverse range of
backgrounds with the ability to listen, engage and empower participants to
achieve their goals in a structured way
- A positive demeanour and resilience in working with challenging behaviour.
- Strong alignment to the Collective Hope Group vision and values
