The Opportunity
We’re inviting experienced Support Coordinator join our team in a contractor role. You’ll retain your independence while gaining access to the support, structure, and systems of a values-driven, registered provider; we’ll give you the tools, support, and space to thrive.
This is a work-from-home role with travel required for client meetings, generally within 30 minutes of your location.

About the Role
You’ll support a caseload of NDIS participants with Level 2 funding, helping them to:
- Understand and use their NDIS plans
- Access services and coordinate supports
- Build skills for independence and self-advocacy
- Navigate crises and reduce barriers to participation
- Track progress and prepare reports for plan reviews
- Strengthen formal and informal support networks
You’ll be part of a team that genuinely values autonomy, collaboration, inclusion and excellence – and that works hard to support you while you support others.
What You’ll Bring
- Relevant tertiary qualification (e.g., Social Work, OT, Psychology, Community Services)
- Minimum 2 years’ experience in Support Coordination or case management
- Strong knowledge of the NDIS and disability sector
- Excellent communication, planning, and problem-solving skills
- Confidence using digital tools in a remote environment
- A values-driven, participant-centred approach
