About the Role

As a Registered Nurse – Community Care, you will take the lead in delivering and overseeing in-home nursing services under the Support at Home Program and the National Disability Insurance Scheme.

This varied role combines hands-on care with leadership and clinical governance responsibilities. You will:

  • Deliver high-quality in-home nursing care
  • Conduct comprehensive client assessments
  • Develop and implement individualised care plans, including for complex clients
  • Provide clinical advice and support incident response
  • Contribute to policy and procedure development
  • Lead, mentor and support Enrolled Nurses across your region

What We’re Looking For

Essential:

  • Bachelor of Nursing
  • Minimum 5 years post-registration experience
  • Current AHPRA registration
  • Current QLD C Class Driver’s Licence

Desirable:

  • Experience in community or aged care nursing (3+ years preferred)

You will also bring:

  • Strong clinical assessment and care planning skills
  • Confidence managing complex client needs
  • High-level written communication and reporting skills
  • Ability to build effective relationships with a range of stakeholders
