About the Role

This is more than a nursing role — it’s an opportunity to lead meaningful change in community-based dementia care.

As the Registered Nurse – Dementia Together, you will deliver high-quality, person-centred care to clients in their own homes while supporting best practice across the organisation.

Key Responsibilities:

  • Deliver compassionate, in-home nursing care and clinical assessments
  • Develop and implement personalised care plans for clients with complex needs
  • Provide clinical guidance and support to internal teams
  • Contribute to clinical governance and continuous improvement initiatives

About You

You are a passionate and experienced nurse who is committed to improving outcomes for people living with dementia.

Essential:

  • Bachelor of Nursing with current AHPRA registration
  • Minimum 5 years’ nursing experience
  • Current Queensland driver’s licence

Desirable:

  • Experience in community or aged care nursing (3+ years preferred)

You will also demonstrate:

  • Strong clinical assessment and critical thinking skills
  • Excellent communication with clients, carers, and multidisciplinary teams
  • High-quality documentation and report writing abilities
