About the Role
This is more than a nursing role — it’s an opportunity to lead meaningful change in community-based dementia care.
As the Registered Nurse – Dementia Together, you will deliver high-quality, person-centred care to clients in their own homes while supporting best practice across the organisation.
Key Responsibilities:
- Deliver compassionate, in-home nursing care and clinical assessments
- Develop and implement personalised care plans for clients with complex needs
- Provide clinical guidance and support to internal teams
- Contribute to clinical governance and continuous improvement initiatives
About You
You are a passionate and experienced nurse who is committed to improving outcomes for people living with dementia.
Essential:
- Bachelor of Nursing with current AHPRA registration
- Minimum 5 years’ nursing experience
- Current Queensland driver’s licence
Desirable:
- Experience in community or aged care nursing (3+ years preferred)
You will also demonstrate:
- Strong clinical assessment and critical thinking skills
- Excellent communication with clients, carers, and multidisciplinary teams
- High-quality documentation and report writing abilities
