About the role:

As a Care Partner, you’ll manage a portfolio of clients within Community Care programs (primarily Support at Home Packages), acting as their key point of contact and trusted advisor.

You’ll work alongside clients to understand their goals, develop tailored care plans, and coordinate services that support their independence, health, and wellbeing.

You’ll also play an important role in ensuring services are delivered in line with funding requirements, while identifying opportunities to grow and strengthen service delivery.

Key responsibilities

🔹 Build strong, trusting relationships with clients and their support networks

🔹 Complete intakes, assessments, and provide information, support, and referrals

🔹 Develop and review personalised care plans aligned to client goals

🔹 Monitor client progress and adjust supports as needs change

🔹 Coordinate services with internal teams to ensure seamless delivery

🔹 Manage client budgets in line with funding guidelines

🔹 Support clients to exercise choice and control over their care

🔹 Contribute to service growth through proactive client engagement

About you

You’re passionate about supporting people and bring a strong understanding of community care and case management.

Essential:

🔹 Minimum 2 years’ experience in Aged Care, Disability, or Community Care

🔹Experience developing care plans and supporting client goals

🔹 Current QLD Driver’s Licence

Desirable:

🔹Formal qualification in Case Management or related field

The successful candidate will also be required to demonstrate entitlement to work in Australia, and to disclose any pre-existing injury or disease which may be adversely affected by undertaking the inherent requirements of the position.
