About the role

We are seeking a dedicated and proactive Relationship Coordinator to lead the coordination and implementation of Mental Health Multidisciplinary Team (MDT) services. This newly created part-time position is responsible for supporting the planning, design and commissioning of the mental health MDT model of care to meet the ACT community needs. This includes coordination and management of contracts in accordance with program objectives.

About the team

Based on the needs of the ACT community, Capital Health Network (CHN) commissions innovative and evidence-based services for people at-risk of poor health outcomes. Reporting to the Service Relationship Manager Primary Care, this position will work within the Commissioning Team that is responsible for the commissioning of both primary care and community services. The position collaborates closely with CHN-funded health service providers across priority areas, including mental health, aged care, alcohol and other drugs, and people at-risk of poor health outcomes.

In this newly created position, you will play a pivotal role in supporting individuals with complex mental health needs in key contracts, evaluation and relationship management role. To deliver services that meet the ACT community needs, you will work proactively to facilitate collaboration across disciplines and ensure a seamless integration of multidisciplinary teams into primary care environments. You will use your strong relationship management skills to collaborate effectively with primary care providers, including general practice and Aboriginal Community Controlled Health Organisations (ACCHOs), as well as mental health practitioners and providers.

A successful candidate will have a demonstrated understanding of issues and key policy developments impacting primary care and mental health in the ACT, and ability to manage projects, contracts, and service delivery within the primary health or community setting.

This position is based in our newly renovated and modern office, located in the heart of the Belconnen town centre. We support flexibility and a hybrid work model at CHN and are happy to work with the successful candidate to ensure balance between work and life.

Position Salary: $48.52 per hour (approximately $96,178 per annum, pro rata to full-time equivalent)

Employment type: Part-time (19 hours per week) fixed term until 30 June 2028

Working Days/hours: Negotiable
