Community Exercise Physiologist

More about the role:

  • In this role as a Community Exercise Physiologist, you will be responsible for delivering evidence-based clinical care in the community through various programs and funding streams (CHSP, HCP).
  • Work closely with the community team including plan managers, Occupational Therapists, Physiotherapists, Osteopaths, Chiropractors, Exercise Physiologists to provide comprehensive care.
  • Assess the clients and complete risk assessments to determine their suitability for exercise programs

Skills:

  • Strong clinical skills in delivering evidence-based Exercise Physiology interventions.
  • Ability to develop or facilitate a care plan and exercise program, as required
  • Communicate clearly and respectfully with Consumers/Participants
  • Develop exercise rehabilitation plans based on the assessment findings
  • Keep detailed and compliant notes within the set time frames
  • Working collaboratively with the support teams such as Care Managers, NOK, advocates etc.
  • Aiming to maximise Consumer/Participant’s independence and assisting them with reaching their goals, whilst respecting their right to choose their treatment plan
  • Regularly reviewing treatment plans and adjusting accordingly

Qualifications:

  • Relevant qualification in Exercise Physiology
  • Minimum of 2 years’ experience
  • Current Police Check
  • Current drivers licence
  • Current Covid Vaccinations
  • Eligible to work permanently in Australia
