About the Role
As a Community Registered Nurse, you will provide high-quality nursing services to both Aspire Hub and our NDIS participants in the community. This diverse role includes clinical care, education, and support across areas such as:
- Wound dressing and management
- Health assessments and care planning
- Medication and continence management
- Chronic disease management
- Specialised clinical care (e.g., catheter care, tracheostomy care, enteral feeding)
You will play a key role in supporting participants to live healthier, more independent lives, while also mentoring and guiding our team in line with Aspire Hub’s culture and values.
Primary Responsibilities
- Conducting participant assessments, reviews, and referrals
- Providing clinical guidance and orientation to Operations teams and Lifestyle Assistants
- Monitoring, auditing, and updating participant medication records and critical medical information
- Developing and supporting medication compliance and clinical governance
- Reporting regularly to Service Managers and stakeholders
- Visiting participants regularly to review care and service quality
- Delivering direct clinical support both in supported accommodation and community settings
- Preparing and managing service agreements
Key Requirements
- Experience in the NDIS sector
- Tertiary qualification in Nursing
- Current AHPRA registration
- Demonstrated confidentiality, discretion, and sound judgement
- Competence in using client management systems and general computer skills
- Current QLD Working with Children Check
- NDIS Worker Orientation Module and Screening Check completed
- Valid Driver’s Licence and a registered, insured vehicle
- Understanding of Clinical Governance under the NDIS Practice Standards
