Amity Health are seeking a dedicated Chronic Conditions Administration Support based in our Albany Head Office to join our Operations Team. The Chronic Conditions Administration Support role provides high-quality administrative and operational support to the Integrated Chronic Disease Care (ICDC) program across the Great Southern, South Coastal Goldfields and Southern, Eastern and Coastal Wheatbelt regions. The position supports efficient and compliant program delivery through accurate data management, appointment and referral coordination, and effective administrative systems that enable the care coordination team to deliver services to clients.

Program Overview 

Amity Health delivers the Integrated Chronic Disease Care (ICDC) program on behalf of the WA Primary Health Alliance across the Great Southern, Wheatbelt and South Coastal Goldfields regions. The program supports adults aged 18 years and over who are living with chronic conditions, including cardiovascular disease, diabetes, respiratory disease and morbid obesity, and who require additional care coordination to improve health outcomes. 

ICDC provides structured, GP-referred care coordination to support integrated management of chronic disease. Care Coordinators work collaboratively with General Practitioners and allied health providers to assess needs, facilitate appropriate referrals, support implementation of GP Management Plans and enhance clients’ capacity to self-manage their condition. Services are delivered via in-person and telehealth appointments to ensure access across regional communities. 

About the Role 

This is a meaningful opportunity to make a genuine difference as we support the needs of our community. As a diligent and effective Program Support Officer, you will: 

  • Provide administrative support to the Integrated Chronic Disease Care (ICDC) program across the Great Southern, Southern Coastal Goldfields and Southern, Eastern and Coastal Wheatbelt region
  • Create and maintain streamlined administrative processes to support efficient program delivery and meet contractual requirements
  • Provide administrative support to Care Coordinators, including appointment scheduling, diary management and triage and coordination of client communications.
  • Liaise with clients, General Practitioners, external contractors and clinicians to support coordinated service delivery
  • Contribute to the ongoing review and enhancement of administrative systems and processes to improve efficiency and service delivery.

 

This is a part-time position based in Albany, offering flexible hours across 3 to 4 days per week

About You 

You’re a skilled and efficient Administrator who has a high attention to detail and is service minded. You will bring: 

  • Previous experience in office administration or a similar administrative role.
  • Demonstrated ability to work within set timeframes with strong attention to detail and accuracy.
  • Proven ability to work independently using own initiative and as part of a team.
  • Strong digital literacy, including proficiency in Microsoft Office applications such as Word, Excel and Outlook.
  • Hold a current and valid C Class Driver’s Licence
  • Hold a National Police Clearance that is no older than 6 months from its issue date (or willingness to obtain)
  • Hold a current and valid Working with Children’s Check (or willingness to obtain)
  • Hold a current and valid National Disability Check (or willingness to obtain)
  • Certificate III (or commitment to undertake) in Business Studies or equivalent
