About the Role
The Registered Nurse plays a central role in delivering high-quality, person-centred aged care within the community.
This role combines:
- Direct clinical care
- Comprehensive assessment and care planning
- Chronic disease management
- Clinical supervision of PCWs and AINs
- Governance oversight at the point of care
- On-call clinical availability
The RN provides leadership through practice ensuring care plans are clinically sound, documentation is accurate, risks are identified early, and scope of practice boundaries are maintained. This is not a purely administrative role. It is an active clinical leadership position within a structured, team-based model.
Key Responsibilities
Clinical Assessment & Care Planning
- Conduct comprehensive nursing assessments and reassessments
- Develop and regularly review person-centred care plans
- Manage clients living with chronic disease (e.g. diabetes, hypertension, renal conditions)
- Identify clinical deterioration and escalate appropriately
- Safely administer medications and treatments
Clinical Governance & Oversight
- Provide clear clinical direction to PCWs and AINs
- Ensure care delivery aligns with documented care plans
- Monitor documentation standards and compliance
- Identify and manage clinical risk
- Participate in audits, incident reviews, and quality improvement
Team Leadership & Support
- Provide clinical supervision and mentorship
- Support onboarding and competency validation
- Foster a respectful, accountable team culture
- Contribute to reflective practice and skill development
Holistic & Relationship-Centred Care
- Build trusting relationships with clients and families
- Support dignity, independence, and meaningful engagement
- Respond sensitively to grief, loss, and transitions
Service & Availability Requirements
WRHH operates as a 24-hour nursing-supported service.
- RNs must be available to work across morning, evening, weekend, and overnight shifts as rostered.
- This role includes availability for clinical on-call support, incident response, and escalation guidance.
- Flexibility, reliability, and commitment to community-based practice are essential.
Essential Criteria
- Current AHPRA registration as a Registered Nurse
- Demonstrated clinical assessment and decision-making capability
- Experience supporting clients with chronic disease management
- Strong documentation and digital literacy skills
- Current Police Check or NDIS Worker Screening
- Current Driver’s Licence and reliable vehicle
- Legal entitlement to work in Australia
Desirable
- Experience supporting people living with dementia
- Community or aged care experience
- Experience working in rural or regional settings
- Postgraduate qualifications in aged care or chronic disease
Classification
Remuneration and classification (RN or Senior RN) will be determined based on experience, demonstrated leadership capability, and scope of responsibility.
