About you

You are someone who is passionate about empowering young people as they transition from care into independent living. You bring a strong foundation in youth work or human services (Certificate IV or equivalent) and you’ve proven your ability to build genuine relationships with young people, their carers and families. You understand the unique journey faced by young people leaving state care, including the cultural needs of Aboriginal young people, and you’re committed to supporting them to develop the life skills, confidence and resilience they need. You are comfortable using a case-management approach, developing individual transition plans, capturing case notes, writing reports and you thrive as part of a team, working collaboratively to create safe, engaging and outcome-focused programs. You hold a current Working With Children Check, National Police Clearance, WA driver’s licence, and are eligible to work in Australia.

About the Role

In this role you will take a lead position in providing high-quality casework and support services to young people leaving state care. You will establish rapport quickly with young people, their carers and families, building trust, managing challenging behaviours, and supporting interpersonal growth. Using a case-management model, you will assess each young person’s physical, emotional and social needs and develop appropriate independent living programs that foster self-esteem, resilience and practical life skills. You’ll engage actively with the young people through a range of physical, pro-social, recreational and educational activities to support connection and growth. Day-to‐day you will collaborate with colleagues, carers and external stakeholders to deliver personalised transition plans, maintain accurate case records and reports, and other duties as required to ensure young people leave care with confidence and capability.
