The Opportunity
We are seeking a Care Navigator (T1D) to join our team in Perth on a part-time basis (0.4 FTE). This is a unique, grant-funded 4-year fixed-term contract dedicated to bridging the gap between clinical management and daily life for those living with type 1 diabetes. Working under the guidance of our Head of Health and Community Impact, you will be a specialist guide for individuals and families, helping them build the resilience required for effective self-management and emotional adjustment.
Key Areas of Responsibility

Emotional Adjustment & Navigation: Lead structured sessions to explore the unique T1D journey, using active listening and motivational interviewing to co-design personalised ‘T1D navigation roadmaps’.

Psychosocial Pathfinding: Coordinate warm referrals to our psychology services and provide a supportive bridge for those on mental health waitlists with evidence-based wellbeing resources.

Knowledge Hub Curation: Build on our comprehensive library of resources and map a network of WA providers to ensure accurate pathways to services and programs for those managing a T1D diagnosis.

Governance & Reporting: Ensure accurate data collection and reporting against WA Health grant-funded KPI requirements.

Innovation: Actively participate in reviewing work standards and bring forward ideas to improve our community and clinic experience.
What You Will Bring

Relevant Expertise: Tertiary qualifications or significant experience in Community Health, Social Sciences, Health Promotion, or a related field.

Navigation Experience: Demonstrated experience in service navigation within a health or community setting, preferably with knowledge of the WA health landscape.

Communication Skills: High-level active listening and relationship-building skills, with the ability to facilitate structured sessions while maintaining safe professional boundaries.

Lived Experience: Lived experience with type 1 diabetes or supporting those living with T1D is highly desirable.
