The Bridge is a welcoming, community minded, not‑for‑profit that supports people with disability and anyone who’s faced disadvantage across south‑eastern and northern Melbourne. We’re all about creating real opportunities and helping people reach their goals—whether that’s through employment pathways, NDIS support, respite and recreation, community activities, or building life skills.
As the Senior Support Worker, you’ll be the glue that keeps our Supported Independent Living (SIL) service running smoothly — supporting staff, empowering participants, and ensuring high‑quality, reliable support every day:
- Keep day‑to‑day operations on track so participant routines and supports run consistently.
- Assist with rostering, shift coverage and responding to last‑minute changes with calm, solutions‑focused thinking.
- Communicate clearly with staff to ensure smooth handovers and shared understanding of participant needs.
- Provide hands‑on support aligned with individual goals, NDIS plans, and capacity‑building outcomes.
- Maintain accurate documentation — shift notes, medication records, incidents, daily progress and goal tracking.
- Escalate risks or concerns promptly and ensure safeguarding and behaviour support practices are followed.
- Build strong relationships with families, allied health, and internal teams, contributing meaningfully to reviews and meetings.
- Champion a safe, inclusive, respectful team culture where everyone feels valued and supported.
Please note this role requires a certificate IV in Disability or Community Services (or higher/equivalent).
