We are seeking a Support Coordinator to join our Service Development team. You will work closely with NDIS participants and their families – helping them to understand and implement their plans, connect with the right providers, manage their funding and work toward greater independence. A strong focus of this role is supporting participants to explore the NDIS Home and Living space.

Working Hours: Full time, hybrid working after an initial 3 month in-office period.

Key Responsibilities:

  • Help participants understand and implement their NDIS plans.
  • Connect participants with providers that fit their actual needs.
  • Support accommodation exploration, co-design and transition.
  • Monitor budgets and keep participants informed about their funding.
  • Chair MDT meetings and coordinator across stakeholder teams.
  • Prepare documentation for plan reviews and NDIA submissions.
  • Maintain accurate case notes and billing records.

Required:

  • Working knowledge of the NDIS – plans, funding categories and how the system operates.
  • Experience in the disability sector, or a genuine drive to build a career here.
  • Strong organisational skills and the ability to manage a varied caseload.
  • A person-centred approach – you listen well and empower others.
  • Clear communicator who builds trust with participants, families and providers.
  • Valid driver’s licence and willingness to travel to client meetings.

Qualifications and Checks:

  • NDIS Workers Screening Check
  • NDIS Working with Children Check
  • Current and valid work rights in Australia
  • Current First Aid and CPR Certificates (or willingness to obtain)
  • COVID-19 Vaccination certificate
  • NDIS Worker Orientation Module (or willingness to obtain prior to commencement)
  • Valid Australian Driver’s License
  • Understanding of the UN Convention on the Rights of Persons with Disabilities and NDIS Code of Conduct
