As a Mental Health and Disability Mentor at Sunflower Services, your responsibilities will include engaging clients in community activities, maintaining accurate case notes, facilitating therapeutic conversations, and assisting clients in achieving their NDIS goals. You’ll work collaboratively within a multidisciplinary team to ensure the best outcomes for our clients while advocating for their rights and autonomy.

Examples of your job roles include:

  • Domestic duties including assistance with light cleaning, cooking, meal preparations and transport.
  • Implementing data for allied health professionals and data collection 
  • Uplifting, calming, trauma-informed and positive conversations 
  • A commitment to helping Participants achieve their NDIS goals
  • Following the Case Management Plan 
  • Following a Behavioural Support Plan 
  • Working within a multidisciplinary team of passionate individuals
  • Maintain professional documentation, incident reports, and participant progress notes
  • Working closely within a multidisciplinary team of passionate individuals including the clinical team, House Managers, families, and guardians.

To succeed in this role, you’ll need to demonstrate experience in mental health, hold relevant certifications such as vaccination certificates, WWCC, police clearance, and NDIS Worker Screening Check, and possess a valid driver’s license with access to your own car and comprehensive car insurance.
