The role
The successful applicant will join a welcoming and committed counselling service located in Rockingham. The role will be to support the initial triage assessment for clients accessing South Coastal Health & Community Services mental health services, as well as providing case management, advocacy, and counselling support as needed. The role involves both individual face to face counselling, telehealth, and engagement with external agencies.
You will have access to in-house clinical supervision to support professional and ethical practice and are well supported by clinic administration and reception staff. You will work closely with the multidisciplinary clinical team and contribute to training, audit and service development with opportunities to participate in specialist research projects.
Your Skills, Knowledge, Abilities and Experience
- High degree of knowledge related to reflective practice, professional guidelines and ethical frameworks.
- Autonomy, enthusiasm, strong communication and clinical skills.
- Experience using a range of assessment approaches and able to formulate and treatment plan considering biopsychosocial factors, theoretical and therapeutic models and structural, historical and developmental processes relevant to the individual, family or group.
- Demonstrated understanding and knowledge of trauma informed care and practice, including an understanding of family and domestic violence, and the impacts of trauma consistent with best practice theoretical frameworks
- Understanding and appreciation of issues for Aboriginal and Torres Strait Islander people and people from CALD background.
- Highly developed interpersonal and communication skills both written and verbal, organisation skills and ability to maintain accurate and confidential client records, record statistical data information and prepare professional reports.
- Knowledge of other relevant services, community resources and experience with connecting clients with appropriate community resources, specialists and services.
Your responsibilities
- Comprehensive triage and assessment of new referrals based upon the appropriate use, interpretation and integration of data from a variety of sources including semi-structured interviews, self-report measures and psychometric measures, to determine eligibility to the service.
- To undertake risk identification and management with clients and to provide clear communication with referrers and primary care in management of clients at risk of harm to themselves and from others.
- Apply a case management approach to assist clients to access support from other health and community services as needed.
- Provision of crisis and brief/short-term interventions and counselling for clients requiring immediate support.
- To maintain an independent caseload of 4-6 clients per day and appropriate professional clinical records and correspondence including assessments, care plans, on-referrals, communication related to risk (assessment and safety plans), discharge and summary reports.
- To participate in team meetings and multidisciplinary clinician meetings, engage in service development including development of client resources, development and delivery of training and audit.
- Engagement in regular clinical supervision and CPD
Your qualifications and/or other requirements
- Social Worker
- Eligible for a Medicare provider number.
- Working with Children Check.
- National Police Clearance (6 months from date of issue)
- Certificate of immunisation
- (Desirable) Current unrestricted Australian drivers’ licence and ability to work across sites in the Metropolitan area.
