Ramahyuck District Aboriginal Corporation (RDAC) was established as an Aboriginal Community Controlled Organisation in 1992 to improve the social, health and economic outcomes for Aboriginal people. RDAC is a leading provider of primary health care and related social and family support services to Aboriginal people and communities across Wellington Shire, Latrobe Valley & Baw Baw Shire regions.
About the role
$ SCHADS 2 $
Fulltime 38 hours
HACC-PYP is for Aboriginal people aged under 50 who need
assistance with daily activities, including personal care, dressing, preparing
meals, house cleaning, property maintenance, community access and using public
transport. The HACC-PYP Assessment Officer will undertake assessments for the Home and Community Care (HACC) Program for Younger People (PYP) to determine eligibility for services.
This role ensures clients receive culturally appropriate, person-centred support that helps maintain independence and engagement within the community. The role will also lead and participate in community development and social inclusion activities. Home based Assessments for the HACC PYP must be completed within Department of Families, Fairness and Housing (DFFH) and Home and Community Care Program for Younger People (HACC PYP) guidelines. This includes supporting younger people with a disability to access government funded services or link into other government programs such as the NDIS appropriate to their needs.
