Behaviour Support Practitioner

Summary:

We are seeking a highly motivated and experienced Social Worker to join our team. The successful candidate will be responsible for providing support to individuals, helping them to develop coping strategies and improve their quality of life. The role requires a compassionate and patient individual who is able to work collaboratively with clients, families, and other healthcare professionals.

Responsibilities:

• Conduct assessments to identify risk, needs and life goals
• Support Behavior Support Practitioner to implement behavior support plans
• Provide professional support and social intervention to clients, families, and caregivers
• Monitor progress and adjust support intervention needs as necessary
• Collaborate with other healthcare professionals to ensure a holistic approach to care
• Maintain accurate and up-to-date records of client progress and interventions
• Attend team meetings and participate in case conferences as required
• Uphold professional demeanor at all times

Qualifications:

• Post Graduate degree in, social work
• Minimum of 2 years’ experience working within community health
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Compassionate and patient approach to working with clients
