About the role
We are seeking a compassionate and proactive Enrolled
Nurse to join our team as a Client Care Partner.
This is a predominantly office-based clinical coordination
role focused on supporting high-quality, person-centred care for our aged care
clients. You will conduct assessments within your scope of practice, identify
clinical risks, monitor care delivery, and collaborate closely with field staff
to achieve the best possible outcomes for clients.
This role is ideal for an experienced EN who enjoys care
planning, clinical coordination, and building strong relationships — while
still having opportunities to provide occasional hands-on support in the
community.
Your responsibilities will include:
- Coordinating client support by conducting assessments, reviewing services, and creating care plans within scope of practice
- Collaborating with clients to prepare individualized budgets using allocated Support at Home funding and reviewing them as needed.
- Continuously monitoring and reassessing client needs, updating Care Plans to reflect any changes, and liaising with field staff to ensure those changes are communicated and actioned.
- Liaising with the multi-disciplinary team regarding client care to ensure a holistic approach and seamless coordination across all services.
- Ensuring data management by collecting, inputting, and reporting on service provision and activity data, while maintaining a high standard of data accuracy and integrity.
- Providing occasional direct client care support, including remote care coordination, or attending on-site, if necessary, alongside other office-based clinical tasks.
What we’re looking for:
- 3–5 years’ experience as an Enrolled Nurse (community nursing desirable)
- Background in risk assessment skills within the scope of an Enrolled Nurse
- Competence in recognising client deterioration and implementing strategies
- Strong communication, interpersonal, and advocacy skills.
- Excellent communication, organisation and time management skills
- Ability to work both autonomously and collaboratively
- Highly organised with strong critical-thinking and problem-solving abilities.
- Passionate about delivering high-quality, person-centred care.
What we need:
- Current AHPRA registration
- Current National Police Clearance
- An unrestricted South Australian Driver’s License
- Working with Children and NDIS clearances
- Current certificates in manual handling, First aid, and CPR
- Proof of mandatory vaccinations, including COVID-19 and flu
