Position Overview

We are looking for a self-motivated, dynamic clinician to join our team as a Care Coordinator who can add value to the organisation by completing the below:

  • Liaise with primary care providers to support the implementation of Management plans for chronic disease.
  • Arrange and facilitate access to allied health services
  • Provide education and support clients and their families, empowering them to manage their conditions effectively.
  • Encourage client engagement and positive relationships.
  • Develop knowledge or relevant local clinical and social services available.
  • Able to utilise and excel in telehealth and face to face appointments.
  • Maintaining accurate and timely client records and documentation in line with organisational and funding requirements
  • Working within clinical governance frameworks, including quality improvement, risk and incident management processes

    What We Are Looking For

    To be successful in the role, you will possess the skills and experience below:

  • Tertiary qualification in Nursing or Aboriginal Health Practitioner.
  • Current registration with AHPRA or relevant professional governing body.
  • Minimum 2 years’ experience in Care Coordination.
  • Strong communication and interpersonal skills.
  • Knowledge of healthcare regulations and standards.
  • A current WA driver’s license
  • A clear police check
  • WA Working with Children Check (WWC)
