We have three outstanding aged care facilities in Vaucluse, Little Bay and Warrawee – Vaucluse incorporating independent living.

The Opportunity

The Clinical Care Coordinator is located at our Vaucluse facility and is responsible for completing all clinical assessments on admission, communicating with nominated treating doctors and families of our residents, managing the process from the day of admission until all assessment processes have been completed.

Roles and responsibilities include but are not limited to:

clinical handover and assess for new new residents on admission.

Formulate Care Plans on admission and periodically review/update care plans in accordance with changes in the resident’s condition as required by legislation.

Actively monitor and coordinate individualised assessments and care planning to optimise the health and wellbeing of residents and reporting on any change of health.

Lodge funding instrument claims within legislative timeframes, ensuring maximum optimisation

Ensure the comfort, privacy, dignity, and preferences of each resident are respected and maintained. Encourage and support residents to make their own decisions.

Provide updates to families on resident condition and respond to concerns as required.

Liaise with medical practitioners and allied health care workers in consultation with other care staff for the provision of services to meet residents’ needs.

Accompany and assist Medical Practitioners as required and record/implement medical orders as requested.

About You

Qualified Registered Nurse with at least 3+ years’ experience in Aged Care (current AHPRA Registration).

Demonstrated experience in assessing, care planning and evaluating complex care needs.

Solid understanding of Aged Care Quality Standards and Principles and other aged-care related legislation.

Demonstrated understanding and application of the government funding instrument.

Intermediated Microsoft Office and computer skills
