Description

We are seeking a highly organized and compassionate Case Manager to join our team. The Case Manager will be responsible for managing and coordinating the care of clients, ensuring that they receive the appropriate services and resources to meet their needs. The ideal candidate will have excellent communication skills, be able to work independently and as part of a team and have a strong commitment to client advocacy.

Responsibilities:

  • Conduct in-home assessments to develop tailored, person-centred support plans in partnership with clients.
  • Oversee the delivery of services that align with clients’ goals, ensuring their evolving needs are met.
  • Collaborate with Life Care’s clinical and allied health teams to provide best-practice care.
  • Monitor clients’ health and wellbeing, identifying when further assessments or clinical referrals are necessary.
  • Ensure compliance with aged care standards and manage client budgets effectively.
  • Liaise with clients’ families and support networks to achieve positive outcomes.
  • Advocate for clients and ensure that their rights are protected
  • Maintain accurate and up-to-date client records

What we are looking for:

  • Excellent communication and interpersonal skills.
  • Strong time management and prioritisation abilities.
  • Demonstrated empathy and flexibility in resolving sensitive issues.
  • Ability to maintain confidentiality and handle sensitive information
  • Proficiency in Microsoft Office and experience with Client Management Systems.
  • Knowledge of aged care standards, S@H, and CHSP is highly desirable.

Qualifications:

  • Minimum of 2 years of experience in case management or a related field
  • National Police Clearance (Aged Care)
  • Current Driver’s Licence
