About the Role

As either a RN Coach or Injury Nurse Coordinator, you will:

Coordinate care for individuals navigating injury recovery, chronic conditions, post-hospital transitions, or integrated care programs via phone.

Empower patients with the knowledge and tools to manage their recovery holistically.

Liaise with key stakeholders (insurers, healthcare providers, and medical teams) to ensure a timely and sustainable return to health.

Close potential gaps in care by providing health education, reviewing discharge instructions, arranging follow-up services, and addressing future health needs.

Work within a multidisciplinary team to improve the quality of healthcare services.

What We’re Looking For

✔️ Current unrestricted AHPRA Registered Nurse

✔️ Minimum 3 years of post-graduate nursing experience in an acute clinical setting

✔️ Strong care management skills and experience coordinating care

✔️ Excellent telephone communication skills and experience delivering telehealth support

✔️ Tech-savvy with confidence using digital platforms and electronic health records

✔️ A passion for improving patient outcomes at a population level

✔️ Ability to work independently and manage time effectively

Desirable

Telehealth, chronic disease management, or care planning experience

Prior experience in injury management, supporting individuals recovering from workplace or motor vehicle injuries

Experience in rehabilitation, return-to-work coordination, or multidisciplinary care

Mental health experience

Autonomous nursing experience

Work Arrangements

Full-time or Part-time with 4 day weeks available (Mon-Fri, 9am – 5pm AWST).

Virtual role – You must have a reliable home internet connection and a private dedicated workspace.
