As we continue to grow we are seeking a highly skilled, experienced, and dedicated Community Registered Nurse to join our team. Experience in providing support at home is highly desired, as it allows you to make a meaningful impact on patients’ lives in their own environment.

What We’re Looking For

  • Current AHPRA registration as a Registered Nurse
  • Experience in support at home is highly desirable
  • Minimum 3 years of clinical nursing experience, with demonstrated capability in community or aged care settings
  • Strong experience in wound care, catheter care, and complex case management
  • Proven clinical leadership and mentoring skills
  • A current driver’s licence and access to a reliable vehicle
  • Current National Police Check or willingness to obtain
  • Unrestricted work rights in Australia
  • Excellent communication and documentation skills
  • Commitment to best practice, holistic care, and continuous improvement
  • Must be able to commit to a full-time work week
  • Currently reside in Darwin, NT, or have plans to relocate to Darwin.
  • Knowledge of the Coordinated Veteran Care Program is a plus

Key Responsibilities

Complex Wound Care

  • Conduct thorough wound assessments (including photography, measurements, and staging)
  • Develop and implement individualized wound management plans
  • Evaluate dressing types, advanced therapies, and negative pressure wound options

Catheter Management

  • Insert, maintain, and troubleshoot indwelling (IDC) and suprapubic (SPC) catheters
  • Educate patients and carers on proper catheter care, infection prevention, and early signs of complications

Patient Assessment & Care Planning

  • Perform holistic in-home assessments (covering physical, psychosocial, and environmental factors)
  • Develop and review person-centered care plans in collaboration with allied health and medical teams
  • Coordinate referrals to relevant health and community services

Clinical Leadership

  • Mentor and support junior nurses and student placements in the community
  • Lead and contribute to the development of clinical policies, procedures, and best-practice guidelines
  • Participate in clinical audits and quality improvement initiatives
