About the Role 

We are looking for an experienced Support Coordinator who will be responsible for providing comprehensive support coordination to a caseload of NDIS funded participants with complex needs, behaviours of concern, medical needs or other high risk circumstances or where the National Disability Insurance Agency has deemed it reasonable and necessary to fund the Support Coordination support in their plan. 

Support Coordinators have a critical role in providing high quality, safe and effective supports, which assist all participants to build capacity to enhance their quality of life and maximise their ability and opportunity to exercise choice and control.  Exercising strong interpersonal skills and qualified expertise you will collaborate with participant and families/ carers and other stakeholders to deliver coordinated, innovative and person-centred support.  

This role is performed in a competitive market environment where customer satisfaction with service quality and outcomes is essential for customer retention and attraction and business sustainability. 

Essential Requirements 

  • Previous experience in a NDIS Support Coordinator role or NDIS Self Management experience. 

  • Strong knowledge of the NDIS price guide and the role of the Support coordinator. 

  • Experience work with (or personal experience) with people living with a disability or mental health. 

  • Experience in planning, implementing and evaluating innovative evidence based NDIS supports. 

  • Demonstrate ability to support a coordinate approach by working with a range of stakeholders to improve outcomes as agreed with the participant. 

  • Excellent written and verbal communication skills. 

  • Ability to present complex information in an easily understood and accessible manner. 

  • Build participants relationships and community supports in line with NDIS choice. 

  • Demonstrate responsiveness to participant needs and wants (internal and external). 

  • Ability to write clear, concise and informative reports. 

  • Knowledge of current and pending legislative changes. 

  • This is an onsite role based at our Head Office in Seymour
