Gidgee Healing is an Aboriginal Community Controlled Health Service committed to improving the health and wellbeing of Aboriginal and Torres Strait Islander people across Northwest and Lower Gulf Queensland. We deliver holistic, culturally safe care through community participation and strong care coordination.
About the Role
As the Elder Care Connector, you will:
- Conduct community outreach to promote aged care services.
- Engage with Elders and families to raise awareness of aged care entitlements.
- Support clients in identifying their goals and accessing appropriate services.
- Assist with My Aged Care registration and review processes.
- Advocate for clients and help identify service gaps in the region.
- Work closely with the Aged Care Support Coordinator and internal teams.
What You’ll Bring
- Certificate in Community Services or relevant experience.
- Strong relationships with local Aboriginal and Torres Strait Islander communities.
- Excellent communication and interpersonal skills.
- Understanding of person-centred care and trauma-informed practice.
- Experience working with older people and knowledge of aged care services.
- Ability to work independently and as part of a team.
Additional Requirements
- Current QLD C Class Driver’s Licence.
- Satisfactory police check and current Blue Card (or willingness to obtain).
- Flexibility to travel and stay overnight in remote areas, including travel by road and small plane.
