At Country Home Services, we’ve been supporting older people in our regional communities for over 35 years, providing care that’s personal, respectful and truly makes a difference. Country Home Services is expanding service offerings under the new Support at Home (aged care) program. We are looking for qualified Clinical Care Partners who are compassionate, organised, and committed to helping older people live well at home.

About the Role:

As a Clinical Care Partner, you will use your clinical expertise and compassionate caring skills to ensure older people with complex care needs receive trauma-informed, person-centred care in line with our values of trust, integrity, teamwork, respect, caring and best practice.

With a Bachelor of Nursing degree, solid experience, and a passion for community care, you will influence continuous improvement and best practice in a meaningful and measurable way by leading and mentoring others.

Your responsibilities will include:

  • Conducting thorough clinical assessments and complex case reviews
  • Reviewing care plans to align services with client needs and preferences within budget parameters.
  • Leading communication and conferences with clients, families, and health professionals to create positive and collaborative team-based, person-centred client care outcomes.
  • Monitoring care transitions, addressing clinical concerns, monitoring health outcomes and escalating to GPs or other allied health specialists appropriately.
  • Mentoring and guiding other Care Partners in case management for individuals with evolving needs.
  • Ensuring compliance with aged care legislation and quality standards.
  • Leading or engaging with incident reviews, standardisation, quality, risk and continuous improvement initiatives, CPD and training activities.

About You:

  • Current AHPRA registration in Nursing, Allied Health, or a related field.
  • Strong understanding of aged care standards and legislation.
  • Demonstrated experience in care planning, case management, and clinical decision-making.
  • Experience in complex case management, preferably in aged care or community settings.
  • Strong communication, cultural competency, and empathy when interacting with clients and their families.
  • Excellent communication and mentoring skills.
  • Valid Driver’s Licence and reliable vehicle (km reimbursement).
  • First Aid/CPR, National Police Check, Australian Work Rights (no visa sponsorships available)
