At Connect Home Care, we deliver high-quality, person-centred supports that empower individuals to live safely, independently, and with dignity. Due to continued growth across the Central Coast, we are seeking experienced Support Workers with a strong background in complex behaviours.

The Role

You will support participants in Supported Independent Living (SIL) and community settings, providing

consistent, safe, and meaningful care.

Key Responsibilities

  • Deliver person-centred supports aligned with care plans
  • Implement Behaviour Support Plans (BSP)
  • Support with personal care and daily living tasks
  • Assist with community access and engagement
  • Complete shift notes and incident reporting
  • Work collaboratively with families and allied health teams

What we’re looking for

  • Experience with challenging behaviours / complex needs
  • Understanding of positive behaviour support
  • Ability to remain calm in high-pressure situations
  • Strong communication and teamwork skills
  • Reliable, flexible, and passionate about making a difference

Essential Requirements

  • NDIS Worker Screening Check
  • Working With Children Check (if applicable)
  • Current First Aid & CPR
  • Driver’s Licence and reliable vehicle
  • Cert III/IV in Disability (preferred)
