CoAct is a not-for-profit organisation with over 25 years’ experience supporting people, particularly those facing disadvantage, into meaningful, sustainable employment. We operate a collaborative network of more than 100 community‑based service providers across metropolitan, regional and remote Australia, including Aboriginal and Torres Strait Islander communities, partnering with individuals and employers to deliver positive, long‑term outcomes.
As part of CoAct’s exciting organisational growth strategy, led by an experienced NDIS General Manager, we are expanding beyond traditional employment services and building a new NDIS business. This includes Support Coordination and Allied Health services, with future plans to open a high school to better support individuals through wellbeing, education and pathways to employment.
Our Support Coordination team will play a key role in delivering inclusive, NDIS person‑centred services that empower individuals to achieve meaningful, sustainable outcomes.
The Role
CoAct is seeking a Support Coordinator – Practice Lead to join our new NDIS Support Coordination team. This role delivers high‑quality, person‑centred NDIS Support Coordination services to participants with complex and multiple support needs, while also supporting strong practice, consistency and quality across the team.
The role supports participants to build capacity, exercise choice and control, and navigate the NDIS and broader service systems. In addition, the Practice Lead contributes to effective service delivery by providing practice guidance, supporting quality outcomes, mentoring team members, and ensuring services are delivered in line with NDIS Practice Standards, safeguarding expectations and organisational requirements.
The Support Coordinator – Practice Lead operates with a high degree of autonomy, managing complex caseloads, coordinating multi‑agency supports, and contributing to a culture of accountability, quality and continuous improvement.
In this role, you will:
- Deliver high‑quality NDIS Support Coordination services to participants with complex and high‑risk needs
- Support participants to understand and implement their NDIS plans, promoting choice, control and informed decision‑making
- Coordinate supports to maximise positive participant outcomes, value for money and plan effectiveness
- Manage and support complex and escalated cases, including proactive risk identification and mitigation
- Provide practice guidance, mentoring and support to Support Coordinators to strengthen service quality and consistency
- Contribute to quality, compliance and documentation standards across the team
- Build and maintain strong relationships with service providers and key stakeholders
Skills and Experience
To be successful in this role, you will bring:
- Demonstrated experience delivering NDIS Support Coordination to participants with complex and multiple support needs
- Sound knowledge of the NDIS, NDIS Practice Standards, safeguarding requirements and service systems
- Experience managing complex or high‑risk cases, including proactive risk identification and escalation
- Strong time management and workload prioritisation skills
- Ability to build and maintain effective, professional relationships with participants, service providers and stakeholders
- Experience supporting, mentoring or guiding colleagues to achieve quality outcomes
- A values‑driven approach, with a strong commitment to person‑centred, ethical practice
Qualifications
- Relevant tertiary qualification (e.g. Social Work, Community Services, Human Services or a related discipline), or equivalent experience within disability, community or social services
Location
This role is based at our Stones Corner office and operates under a hybrid work‑from‑home arrangement.
