Aged Care Clinical Assessors | Full-time & Part-time opportunities available!
About You:
We are seeking dedicated Aged Care Clinical Assessors to join our multidisciplinary health team, working across Melbourne’s Southern & Eastern suburbs.
In this role, you’ll deliver aged care assessment services to older Australians, enabling them to remain living independently at home.
Preferred candidates will have:
- Experience conducting home support or comprehensive aged care assessments
- Ability to work independently and as part of a team
- High-level computer literacy
- Strong problem-solving skills and ability to follow relevant guidelines
- Current Australian driver’s license with a reliable, fully insured vehicle
- Valid National Criminal History Check and Working with Children Check (or willingness to obtain)
What You’ll Do:
- Conduct face-to-face assessments to determine eligibility for My Aged Care supports
- Review previously assessed clients
- Develop client-centered support plans using a wellness and reablement approach
- Liaise with referral sources and community organisations to secure supports for clients
- Ensure timely, accurate documentation via the My Aged Care online portal
