Summary:

The Care Coordinator will be responsible for coordinating patient care services, advocating for patient needs, and ensuring effective communication between healthcare professionals, patients, and their families or caregivers.

Responsibilities:

  • Coordinate and manage patient care services based on individual needs and resources
  • Conduct comprehensive assessments and develop individual care plans
  • Collaborate with healthcare professionals and other care coordinators
  • Advocate for patients’ access to necessary medical services and resources
  • Schedule and monitor appointments, referrals, and care transitions
  • Communicate with patients and families to provide guidance and education on disease management and self-care strategies
  • Maintain accurate patient records and care plans
  • Participate in team meetings and quality improvement initiatives

Qualifications:

  • Certificate IV or Bachelor’s degree in Healthcare Administration, Nursing, Social Work or related field
  • Minimum of two years of experience in healthcare or care coordination
  • Knowledge of healthcare delivery systems and care coordination techniques
  • Excellent organizational, communication, and interpersonal skills
  • Ability to work independently or as part of a team in a fast-paced environment
  • Strong analytical and problem-solving skills
  • Commitment to patient-centered care and empathy for patient needs
  • Proficiency in medical terminology, electronic health records, and Microsoft Office Suite
