Amity Health is seeking a dedicated ITC Care Coordinator to deliver culturally appropriate care coordination services to Aboriginal and Torres Strait Islander people living with chronic health conditions.

In this role, you’ll work closely with General Practitioners, Aboriginal Health Workers, and other health professionals to improve access to primary care, strengthen continuity of care, and support improved health outcomes. You’ll provide proactive coordination across the care continuum, support access to supplementary services, and contribute to building the cultural capability of mainstream primary health providers.

About the Role

As a valued Health Professional, you will:

  • Provide culturally safe, person-centred care coordination to Aboriginal and Torres Strait Islander clients enrolled in the ITC program, supporting improved management of chronic health conditions.
  • Support clients to engage with their GP and relevant health services, assisting them to understand and follow agreed care plans, referrals, and appointments.
  • Identify and address cultural, systemic, and practical barriers to accessing care, and advocate on behalf of clients within mainstream primary care settings.
  • Build and maintain strong working relationships with GPs, Aboriginal Medical Services, allied health providers, and other key stakeholders, and participate in coordinated service planning.
  • Administer ITC Supplementary Services funding in line with program guidelines and organisational policy, ensuring appropriate, equitable, and well-documented use of funds.
  • Maintain accurate client records and program data, meet reporting and contractual requirements, and contribute positively to team meetings, supervision, and service planning.

This role is a Fixed Term Contract for 6 months, part-time in nature (45.60 hours per fortnight) based in Narrogin.

  • Ability to work collaboratively with Aboriginal communities and health professionals
  • Excellent communication, organisation, and record-keeping skills
  • A commitment to improving health outcomes for Aboriginal and Torres Strait Islander people.
