Amity Health is seeking a dedicated Integrated Chronic Disease (ICDC) Care Coordinator to deliver culturally appropriate care coordination services to our clients living with chronic health conditions.

In this role, you’ll work closely with General Practitioners and other Health Professionals to improve access to primary care, strengthen continuity of care, and support improved health outcomes. You’ll provide proactive coordination across the care continuum, support access to supplementary services, and contribute to building the cultural capability of mainstream primary health providers.

About the Role

As a valued Health Professional, you will:

  • Provide culturally safe, person-centred care coordination to our clients enrolled in the ICDC program, supporting improved management of chronic health conditions.
  • Support clients to engage with their GP and relevant health services, assisting them to understand and follow agreed care plans, referrals, and appointments.
  • Identify and address cultural, systemic, and practical barriers to accessing care, and advocate on behalf of clients within mainstream primary care settings.
  • Build and maintain strong working relationships with GPs, allied health providers, and other key stakeholders, and participate in coordinated service planning.
  • Administer ICDC Supplementary Services funding in line with program guidelines and organisational policy, ensuring appropriate, equitable, and well-documented use of funds.
  • Maintain accurate client records and program data, meet reporting and contractual requirements, and contribute positively to team meetings, supervision, and service planning

We are currently seeking a pool of casuals to support with extended leave coverage and a seasonal increase in demand. The appointments are based in Albany with shifts provided as and when required.

You will bring:

  • Experience in care coordination, case support, or service navigation
  • Strong understanding of culturally safe, respectful, and person-centred practice
  • Ability to work collaboratively with vulnerable communities and health professionals
  • Excellent communication, organisation, and record-keeping skills
