Community Support Worker (Companionship and Community Access)

About the role

Allegiance Heart & Home Care is seeking a warm, reliable Support Worker to provide companionship, community access, and emotional support to an older client living with dementia. The role includes accompanying the client on outings and appointments, supporting meaningful engagement, and assisting with light personal care and domestic tasks as required.

Shift available

Wednesday 9:30am to 1:30pm (4 hours weekly)

Key responsibilities:

Companionship and emotional support

Provide friendly, respectful companionship and reassurance Support engagement in conversation and simple activities Use calm prompts and redirection where needed Community access and accompaniment

Accompany the client to local outings and appointments Support safe mobility and supervision during community access Assist with transport as required (your own vehicle) Personal care and daily living support (as required)

Prompting and support with hygiene routines, grooming, and dressing Support safe shower routine if required and in line with the care plan Light domestic assistance, for example tidy up, laundry prompts, and meal or drink preparation

Documentation and communication

Complete shift notes in ShiftCare Report any changes in mood, mobility, or health to the care team promptly

About the client support needs

Client has dementia and benefits from a calm approach, one step prompts, and routine Client requires supervision for safety, especially in the community Arabic speaking background, family support present, and clear communication is important

Essential requirements

Experience providing community access and companionship to older people Comfortable supporting clients with dementia Willing to support basic personal care and domestic assistance as needed Current National Police Check (or willingness to obtain) Current Working With Children Check (or willingness to obtain if required) Manual handling certificate preferred Comprehensive car insurance (mandatory) and a reliable vehicle Valid Australian driver licence Smartphone and ability to use ShiftCare for shift notes

Desirable

Certificate III in Individual Support (Ageing, Home and Community) or working towards Arabic language skills
