What are we looking for?

As a Care Partner, you will work closely with clients and their support networks to onboard them effectively and set meaningful goals and care plans. You will coordinate proactive services using a holistic approach to care that improves the lives of our clients and maximises their independence in their own homes.

As the face of allcare, you will adopt and facilitate a person-centred approach to client care, managing a portfolio of clients across the Illawarra while developing individualised care plans, goals, and budgets.

This is a full-time role and requires flexibility from the candidate to provide support to clients.

The role involves:

  • collaborating with clients, families, and advocates to create personalised care plans.
  • assessing initial intake details and identifying service needs.
  • building strong client relationships and setting goals together empowering choice and control.
  • proactively seeking and responding to client feedback.
  • planning fun and engaging outings and activities for our clients to improve their social connection and community engagement.
  • reviewing and updating care plans for existing clients.
  • managing client budgets and ensuring services are adequately funded.
  • problem solving with the client to ensure they remain safe in their own homes.
  • liaising with roster teams to ensure service schedules are met and any changes for clients are appropriately managed and communicated.
  • ensuring compliance with Aged Care Quality Standards and other industry requirements.
  • promoting allcare as a provider of choice.

To be successful in this role you will have:

  • a Certificate III in Aged Care or equivalent preferred.
  • a minimum 3 years of person-centred planning, case management or client liaison experience in community settings, preferably with Home Care Packages or customer facing roles.
  • knowledge of aged care industry services and standards.
  • strong communication and interpersonal skills.
  • customer service focus and commitment to client needs.
  • initiative in prioritising tasks and following up on client needs.
  • high level of computer literacy and ability to learn new applications.
  • empathy, compassion, and sensitivity to confidentiality.
  • genuine interest and enthusiasm for working with older people.

Candidates will also hold a valid Driver’s Licence and access to a fully insured vehicle, have received a minimum two doses of COVID-19 vaccination, be willing to obtain an annual flu vaccination, be willing to undertake a National Police History check and have valid Right to Work status in Australia.
