The Job:

The Housing First worker will provide intensive case management to Aboriginal individuals and families accessing Entry Point services. They will respond to referrals in a timely manner utilising the Housing First Principles of Practise. Both assertive outreach and office based services will be provided by the Housing First Team. Using a collaborative and planned approach to ensure our clients get the services and supports they need to reach their goals. Using Culturally safe practises, the Housing First Worker will assess client needs (and family where appropriate) and arrange, coordinate and advocate for the delivery and access to a range of programs and services that support the client to reduce the risk of homelessness and/or achieve stable housing.

Tasks will include:

  • Respond promptly and appropriately to walk-in clients seeking support
  • Provide assertive outreach services to rough sleepers, hotspots and to community spaces
  • To respond to referrals within 24 hours by contacting the client or referrer
  • Complete Initial Assessment and Planning Tools (IAP) with client’s and deliver timely, responsive interventions and outcomes
  • Co-ordinate client’s case plans and lead case management planning teams
  • Demonstrate strong teamwork and collaboration skills
  • Work closely with our Multi-disciplinary teams to ensure holistic case management and care

Your Skills & Experience:

  • Welfare work and / or community development
  • Experience in working with Aboriginal people experiencing or at risk of homelessness
  • Demonstrated understanding of the unique health challenges and social issues faced by the Aboriginal community, including the impacts of alcohol and drug use and homelessness.
  • Ability to provide culturally safe and respectful care and services to the Aboriginal community.
  • Leadership skills to lead and inspire a team, fostering a collaborative and supportive work environment.
  • Networking skills to build and maintain relationships with various stakeholders, including local health services, law enforcement agencies, and Aboriginal organisations.
  • Outreach skills to conduct assertive outreach within the community, demonstrating sensitivity and respect for cultural norms and practices.
  • Computer literacy skills including Microsoft Office suite
  • First Aid (desirable)

Mandatory Requirements:

  • Current Victorian Driver’s License;
  • National Police Certificate (no more than 3 years old)
  • Current Victorian Employment Working With Children Check;

Desirable:

  • Certificate IV in Community Services or relevant experience in the community sector
  • First Aid or Mental Health First Aid
  • Aboriginal Cultural Awareness Training
