Summary:
The NDIS Service Coordinator role is a complex case management role, overseeing a caseload inclusive of Supported Independent Living houses and community clients.
The purpose of the role is to guide, mentor and support a team of workers to implement person-centered active supports to create an environment in which individuals can exercise their human rights and work towards achieving their goals, dreams and aspirations.
Responsibilities:
- Work collaboratively with a team inclusive of a service manager, operations manager, and team leaders.
- Oversee the clinical operations of supported independent living houses and teams.
- Ensure that participants and teams are working towards achieving NDIS goals and that supports are focused on building individual capacity.
- Complete NDIS Progress Reports in line with participant NDIS Plan Reviews.
- Data reporting in line with individual plans and regular reporting requirements.
- Lead, coach, supervise and mentor staff on a day-to-day basis to develop skills and knowledge and ensure client needs are met.
- Be responsible for approving and monitoring client activity risk assessments.
- Regularly review and update client documentation to enable safe and effective support.
- Liase with and develop relationships with the team, external stakeholders, client families, allied health professionals and support coordinators e.g through stakeholder meetings and regular client team meetings
- Work collaboratively with participant stakeholders to achieve positive outcomes.
Qualifications:
- Bachelor’s degree in nursing or a related field.
- Minimum of 5 years of experience as an Enrolled or Registered Nurse
- Strong leadership and management skills, with the ability to motivate and inspire others.
- Excellent communication and interpersonal skills, with the ability to work effectively with a diverse group of stakeholders.
- NDIS and community services experience
- NDIS worker screening check
- Strong analytical and problem-solving skills, with the ability to make sound decisions based on data and evidence.
- Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork.
