About the role
Shifting Sands Support Services is an NDIS Registered Provider looking for an NDIS Support Coordinator/Psychosocial Recovery Coach to subcontract with our team. Based in Birtinya, QLD, this role will be responsible for providing high-quality support coordination and recovery coaching services to our NDIS clients across the Sunshine Coast and Moreton Bay Community.
What you’ll be doing
Develop and implement tailored support plans to help NDIS participants achieve their goals
Coordinate and facilitate access to a wide range of community, health and other support services
Build strong relationships with participants and their families to understand their unique needs
Empower participants to increase their independence and social and economic participation
Provide one-on-one psychosocial recovery coaching to assist participants in their recovery journey
Collaborate with allied health professionals and other service providers to ensure holistic care
Maintain accurate records and reporting to ensure compliance with NDIS requirements
What we’re looking for
Relevant minimum Certificate IV qualification in a human services field (e.g. Counselling, Mental Health, Community Services, Case Management)
Extensive experience in NDIS support coordination and/or psychosocial recovery coaching
Excellent communication and interpersonal skills to build trust and rapport with clients
Demonstrated ability to display and maintain strong professional boundaries
Strong problem-solving and critical thinking skills to overcome challenges
Commitment to upholding the rights, dignity and independence of people with disability
Ability to work autonomously as well as collaboratively within a team
Current driver’s licence, Blue Card, Yellow Card and access to a reliable vehicle with relevant insurances
