We have a unique opportunity for an experienced NDIS Support Coordinator to join our team based in our Bendigo office.

This role is a part-time position of 3 days per week.

About the Role

The role of our NDIS Support Coordinator’s is to provide high quality support coordination for our participants and facilitate the implementation of all supports identified in their NDIS plans.

In this role we focus on strengthening a participant’s ability to navigate the NDIS and empower and enable our participants so they can participate more fully in their community of choice. Our NDIS Support Coordinators play an integral role in the development and achievement of our participant’s goals to maximise their independence.

As an NDIS Support Coordinator, you will manage your portfolio of NDIS participants relying on your ability to build rapport and foster relationships, all the while maintaining your support targets. You will use your high-level stakeholder management skills to build and leverage networks to ensure the best support for your customers. You will set up service agreements with providers, prepare plan reviews, conduct risk assessments and critically provide support should issues arise.

Skills and Experience for the Role

  • Demonstrated experience in Support Coordination within the Community Sector
  • Experience in working with complex participants and families
  • Ability to initiate and maintain contacts with a broad range of people and community organisations to ensure our clients’ access to services, appropriate communication with fellow workers, families, carers and other relevant stakeholders
  • Experience and knowledge of the NDIS, a good understanding of associated processes and support networks
  • The ability to deal with changes in the disability and aged care sectors
  • Exceptional verbal and written communication
  • An ability to work efficiently with a CRM database
  • Experience in a KPI-driven environment

  • Current Driver’s license and reliable transportation
  • NDIS Worker Screening Clearance
  • Qualification in Aged Care or Disability
  • Excellent communication and interpersonal skills
