About Us:

Bidgerdii Community Health Services is an Aboriginal and Torres Strait Islander community controlled primary health care service, operating in the Rockhampton and wider Central Queensland region for more than 30 years.

Our services include general practice bulk-billing clinics, social and emotional wellbeing and generalist counselling services, chronic disease management, preventative health screening, children and family health and early childhood education services.

About the Role:

The Aged Care Support Coordinator supports clients to access appropriate local aged care services by providing advocacy, guidance, and culturally safe support. The role helps clients understand their aged care entitlements, registration, costs, and prepares Elders and families for assessments and plan reviews. The coordinator works closely with assessors and the Aged Care Connector, using culturally safe and trauma-informed practices, with duties tailored to each client’s needs.

Duties include. But not limited to:

  • Engage and support clients and families
  • Assist with My Aged Care registration and navigation
  • Explain aged care services, costs, and entitlements
  • Advocate for clients during assessments and reviews
  • Liaise with assessors, service providers, and the Aged Care Connector
  • Coordinate referrals and follow up supports
  • Maintain records and apply culturally safe, trauma-informed practice

About You:

You a compassionate, optimistic individual wanting to make a difference in people’s lives especially our Aboriginal and Torres Strait Islander community and have the following:

  • Relevant Certificate IV or higher qualification
  • Experience working with older Aboriginal and Torres Strait Islander people in health or community care
  • Strong community connections and knowledge of aged care and support services
  • Excellent communication and relationship-building skills
  • Understanding of person-centered, trauma-informed practice
  • Knowledge of My Aged Care and ability to support service access
  • Strong record-keeping, initiative, and problem-solving skills

Practical Requirements:

  • Immunisation requirements – It is a condition of employment for this role for the employee to be, and remain, vaccinated against the following vaccine preventable diseases: Measles, Mumps, Rubella, Varicella, Hepatitis B, Hepatitis A and Pertussis (whooping cough) during their employment.
  • Current CPR and First Aid Certificate.
  • Current Blue Card (Working with Children and young people) or willingness to apply for a Blue Card.
  • NDIS Workers Screening card (Yellow Card) or willingness to apply for a Yellow Card (note a current NDIS Workers Screen card will be required prior to commencement).
  • Current C Class Drivers Licence (QLD)
  • Satisfactory Police Criminal History Check.
