Position Summary
Are you passionate about empowering individuals with disabilities to live their best lives? People First Healthcare, a registered NDIS provider, is looking for a dedicated and experienced NDIS Support Coordinator to join our team.
In this rewarding role, you’ll work closely with NDIS participants, their families, and service providers to develop, coordinate, and monitor tailored support plans. You’ll also be an advocate for PFHC in the community—promoting our services and ensuring positive outcomes for every participant.
Key Responsibilities
• Collaborate with participants and their support networks to create individualised NDIS support plans.
• Promote PFHC within the community as an approved NDIS provider.
• Coordinate service providers and establish service agreements aligned with participants’ goals.
• Support participants in accessing and navigating services under the NDIS framework.
• Monitor progress, review goals, and adjust plans to ensure continued success and alignment.
• Assist participants in preparing for plan reviews.
• Maintain accurate records and documentation on service delivery and outcomes.
• Communicate effectively with participants, families, and stakeholders.
• Act as the key contact for plan reviews, service issues, and conflict resolution.
• Recognise and respond to changes in participants’ mental, physical, and cognitive health.
• Advocate for and uphold the rights and dignity of individuals living with disabilities.
• Promote cultural awareness and inclusivity, particularly for CALD participants.
• Support ongoing service improvement and quality compliance.
What You’ll Bring
• Tertiary qualifications in Disability, Social Work, Psychology, or a related field.
• In-depth knowledge of the NDIS, its operational framework, and policies.
• Demonstrated experience in support coordination or related disability support services.
• Excellent interpersonal and communication skills.
• Strong time management and organisational abilities.
• Commitment to cultural sensitivity, inclusion, and client-centred care.
• Understanding of relevant compliance areas including WHS, NDIS Quality Standards, and infection control.
• A proactive, compassionate, and resilient approach to participant care.
Mandatory Requirements
• Unrestricted right to work in Australia.
• Current National Police Check (or willingness to obtain).
• Working with Children Check (or willingness to obtain).
• NDIS Worker Screening Check (or willingness to obtain).
• Minimum of 2 doses of COVID-19 vaccination.
• Current First Aid and CPR Certificate (or willingness to obtain).
