As a Support Planner, you will play a critical role in coordinating care services, managing participant support plans, and ensuring seamless service delivery under the NDIS and My Aged Care service streams.

We are seeking an experienced, detail-oriented individual who excels at managing rosters and scheduling services across large geographic areas under the SCHADS Award. Your problem-solving abilities and adaptability will ensure that participant needs are met efficiently, even as circumstances change. Excellent time management and logistical planning skills will be essential in delivering seamless high-quality care.

How you contribute to our mission

As a Support Planner, you will be the first point of contact for participants, carers and staff, ensuring they receive timely and effective services. Your responsibilities will include:

• Roster management and scheduling services across broad geographic areas, quickly adapting to changing needs and availability

• Ensuring compliance with NDIS and My Aged Care funding allocations

• Problem-solving daily challenges to ensure seamless service delivery

• Providing after-hours phone support on a rotating roster to ensure continuous care for participants

• Supporting the management of participant support plans and service agreement renewals

• Identifying and advocating for clients’ welfare needs while offering support and guidance to field staff.

Key Requirements

We are looking for someone who thrives in a fast-paced environment, can think on their feet and is highly organised with good attention to detail. Key requirements include:

• 2-3 years of experience in a similar rostering role, ideally in homecare services

• Ability to manage complex rosters and schedules across large, geographically diverse areas

• Excellent communication, customer service, problem-solving, and organisational skills

• Strong understanding of the SCHADS Award is a must

• Demonstrated experience with NDIS and/or My Aged Care (highly desirable)
